If you need hard copies of public information documents, please submit a request to the Board office at (fax) 515-281-7969 or at IDB@iowa.gov. Electronic copies of these archived records delivered by electronic mail, shall be calculated at $ .10 per page; the minimum charge shall be $5.00. A $16.00 per hour fee shall be charged for labor in excess of one-half hour for searching and copying documents or retrieving and copying information stored electronically. The Board shall not require payment if the fee is less than $5 total.

Findings of Fact, Conclusions of Law, Decision, and Order

The Board approved the Findings of Fact, Conclusions of Law, Decision, and Order in the Matter of Jack A. Elder, D.D.S., of Lake City, Iowa. This resolves the Statement of Charges issued by the Board on January 24, 2020.  Dr. Elder's license was placed on probation for a period of five years subject to the terms and conditions established in the Order.

Other Orders

The Board approved a Reinstatement by Consent Agreement with Shawn Kerby, D.D.S.  The order establishes the terms and conditions for the reinstatement of Dr. Kerby's dental license.

The Board issued a Termination Order in the Matter of Todd Becker, D.D.S.

Thomas Cooney, D.D.S., of Indianola, Iowa. The Settlement Agreement & Final Order resolves the Notice of Hearing and Statement of Charges in the Matter of Thomas Cooney, D.D.S. that the Board issued on September 27, 2019.  Dr. Cooney was charged with failure to comply with the standard precautions for preventing and controlling infectious diseases and managing personnel health and safety concerns related to infection control.

Kenneth Wolf, D.D.S., of Nashville, Tennessee. Dr. Wolf was charged with employing or permitting an unlicensed or unregistered person to practice dentistry, dental hygiene, or dental assisting and encouraging, assisting or enabling in any manner the unauthorized practice of dentistry, dental hygiene, or dental assisting.  There is no hearing on this matter, as this is a combined Statement of Charges, Settlement Agreement and Final Order.​

Lyn Rolling, R.D.A., of LeMars, Iowa. Ms. Rolling was charged with unprofessional or unethical conduct.  There is no hearing on this matter, as this is a combined Statement of Charges, Settlement Agreement and Final Order.​

The Board issued a Termination Order in the Matter of Ala'a Al Arabi, D.D.S. The probation placed upon Dr. Al Arabi's dental license has been terminated, and is returned to its full privileges.

The Board entered into a Stipulated Registration Agreement with Joey Brown, R.D.A. Ms. Brown reported multiple convictions on her application for registration.

Robert Lothrop, D.D.S., of Glenwood, Iowa. Dr. Lothrop was charged with failure to fully and promptly comply with office inspections conducted at the request of the Board to determine compliance with sanitation and infection control standards.  There is no hearing on this matter, as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Lisa Heckman, R.D.H., of Gibsonia, Pennsylvania. Ms. Heckman was charged with failure to meet the continuing education requirements for license renewal set forth in 650 IAC 25.  There is no hearing on this matter, as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Dr. Johnson was charged with failure to maintain a satisfactory standard of competency in relation to prescribing and with noncompliance with the minimal sedation standards set forth in 650 IAC 29.7.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Cindy Delaney, R.D.H., of Waukee, Iowa. Ms. Delaney was charged with engaging in the practice of dental hygiene with a lapsed license.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Morgan Gantenbein, R.D.A., of La Crosse, Wisconsin. Ms. Gantenbein was charged with engaging in the practice of dental assisting without a registration.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Roxane Moe, R.D.A., of Decorah, Iowa. Ms. Moe was charged with engaging in the practice of dental assisting without a registration.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.
