If you need hard copies of public information documents, please submit a request to the Board office at (fax) 515-281-7969 or at IDB@iowa.gov. Electronic copies of these archived records delivered by electronic mail, shall be calculated at $ .10 per page; the minimum charge shall be $5.00. A $16.00 per hour fee shall be charged for labor in excess of one-half hour for searching and copying documents or retrieving and copying information stored electronically. The Board shall not require payment if the fee is less than $5 total.

Donald Peterson, D.D.S., of Coralville, Iowa. The Board issued a Notice of Hearing and Statement of Charges in the Matter of Thomas Cooney, D.D.S., of Indianola, Iowa.  Dr. Cooney has been charged with failure to comply with the standard precautions for preventing and controlling infectious disease and managing health and safety concerns related to infection control as required or recommended by the CDC.  A hearing has been scheduled for January 23, 2019.

Settlement Agreement and Final Order

Gregory Garro, Jr., D.D.S., of Omaha, Nebraska. The Settlement Agreement & Final Order resolves the Notice of Hearing and Statement of Charges in the Matter of Gregory Garro, Jr., D.D.S. that the Board issued on April 5, 2019.  Dr. Garro's dental license has been indefinitely suspended until further Order of the Board.  Should Dr. Garro's license be reinstated, the license shall be placed on probation for a period of five years subject to the terms established in this Order.

Jacob Hopper, D.D.S., of Urbandale, Iowa. Dr. Hopper was charged with failure to maintain a reasonably satisfactory standard of competency in the practice of dentistry.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

William Skinner, D.D.S., of Urbandale, Iowa. Dr. Skinner was charged with knowingly making misleading, deceptive, untrue or fraudulent representations in the practice of dentistry, and violating the terms of a recovery contract entered into with the Iowa Practitioner Review Committee (IPRC).  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Other Orders

The Board issued a Termination Order in the Matter of the Reinstatement Order for Catherine Abbott, D.D.S. (formerly Catherine Reno, D.D.S.)  The probation placed upon Dr. Abbott's dental license has been terminated, and is returned to its full privileges.

The Board issued an Order Lifting Restriction in the Matter of the Nicholas Hindley, D.D.S.  The restriction from taking new orthodontic cases is hereby lifted.  Respondent may resume providing orthodontic care.  All other terms and conditions of the Settlement Agreement and Final Order remain in effect.

Mark McConaughy, D.D.S., of Cedar Falls, Iowa. Dr. McConaughy was charged with employing or permitting a person with a lapsed license to practice dental hygiene.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Jack Osterhaus, D.D.S., of Dubuque, Iowa. Dr. Osterhaus was charged with a failure to maintain a reasonably satisfactory standard of competence in the practice of dentistry.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Daniel Thrall, D.D.S., of Cedar Falls, Iowa. Dr. Thrall was charged with employing or permitting a person with a lapsed license to practice dental hygiene.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Howard Trunnell, D.D.S., of Cedar Falls, Iowa. Dr. Trunnell was charged with employing or permitting a person with a lapsed license to practice dental hygiene.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Meliah Rich, R.D.A., of Des Moines, Iowa. Ms. Rich was charged with engaging in unprofessional conduct.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Other Orders

The Board entered into a Stipulated Agreement to Discontinue Practice with Jay Buckley, D.D.S., of Des Moines, Iowa.  Dr. Buckley was informed of a pending complaint against him regarding the Board's allegation of his failure to meet the standard of care.  The agreement provided sufficient time for Dr. Buckley to wind down his practice and to provide continuity of care for current patients, and to meet the Board's interest in protecting the public and resolving the pending complaint.

The Board entered into a Settlement Agreement and Final Order with Phelan Thomas, D.D.S., of West Des Moines, Iowa.  The settlement agreement resolves the charges in the Matter of Phelan Thomas, D.D.S.  Dr. Thomas' license will be placed on probation for a period of five years and is subject to the terms listed in the agreement.

The Board entered into a Stipulated Agreement to Discontinue Practice with George Weber, D.D.S., of Glenwood, Iowa.  Dr. Weber was informed of a pending complaint against him regarding the Board's allegation of his failure to meet the standard of care in providing dental care.  The agreement provides sufficient time for Dr. Weber to wind down his practice and to provide continuity of care for current patients, and to meet the Board's interest in protecting the public and resolving the pending complaint.

The Board issued a Termination Order in the Matter of the Stipulated Registration Agreement for Lindsea Paris, R.D.A.  The probation placed upon Ms. Paris' dental assistant registration has been terminated, and is returned to its full privileges.

Tony D. Stuntz, D.D.S., of Council Bluffs, Iowa. Dr. Stuntz was charged with employing or permitting an unregistered person or a person with a lapsed registration to practice dental assisting.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Laura E. Anderson, R.D.H., of Waterloo, Iowa. Ms. Anderson was charged with engaging in the practice of dental hygiene with a lapsed license.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Teresa L. Keehner, R.D.H., of Guttenberg, Iowa. Ms. Keehner was charged with engaging in the practice of dental hygiene with a lapsed license.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Melanie See, R.D.A., of West Des Moines, Iowa. Ms. See was charged with practicing beyond training.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Hope Wilson, R.D.A., of West Des Moines, Iowa. Ms. Wilson was charged with practicing beyond training.  There is no hearing on this matter as this is a combined Statement of Charges, Settlement Agreement and Final Order.

Other Orders

The Board issued an Order denying a request to terminate the probation in the Matter of Masih Safabakhsh, D.D.S.  The Board previously reinstated the license of Dr. Safabakhsh on August 1, 2014 and placed the license on indefinite probation subject to multiple terms and conditions, including practice monitoring.

The Board issued an Order denying a request to reinstate Dr. Skinner's implant privileges and use an implant monitor in the Matter of William Skinner, D.D.S.  The Board entered into the agreement with Dr. Skinner on July 14, 2017.  The 2017 Order placed the license on indefinite probation and restricted Dr. Skinner from practicing in the area of implant dentistry.
